Mesenchymal stem cells (especially adipose derived) and their secretome actively rebuild and rejuvenate skin. They stimulate collagen and elastic fiber production, enhance dermal density, and improve hydration and capillary density, leading to smoother texture and more youthful appearance (Jo et al., 2021; Surowiecka & Strużyna, 2022). In clinical aesthetic contexts, ADSC rich preparations have been associated with visibly improved skin quality after facial rejuvenation procedures (Surowiecka & Strużyna, 2022).
In wound related aesthetics, MSCs accelerate closure, promote neovascularization, and reduce fibrosis, so scars are often flatter, softer, and more similar to surrounding skin (Wu et al., 2024; Bian et al., 2022; Guillamat-Prats, 2021). Systematic reviews of chronic wounds and soft tissue defects show AD MSCs enhance healing and tissue volume restoration with good cosmetic outcomes and no major adverse events reported (Bian et al., 2022; Gentile & Garcovich, 2021). For hypertrophic and keloid scars, all in vivo studies reviewed showed improvements in macroscopic and histologic appearance after MSC or MSC conditioned media, without complications (Bojanic et al., 2021).
Hair restoration is another positive area: MSC conditioned media and secretome stimulate hair follicle activity and regrowth, with favorable cosmetic results in both preclinical and early clinical work (Zare et al., 2025; Montero‐Vílchez et al., 2021). Across more than 100 studies, MSC conditioned medium improved wound healing, hair restoration, and skin rejuvenation in animals and humans (Montero‐Vílchez et al., 2021).
A particularly encouraging development is cell free MSC therapy: conditioned media, secretome, and extracellular vesicles. These products reproduce many of the regenerative, pro-angiogenic, and anti-inflammatory benefits of live cells while reducing risks related to cell engraftment or immune reactions (Zare et al., 2025; Bian et al., 2022; Jung et al., 2024; L et al., 2019). Reviews emphasize their lower immunogenicity and diminished risk of tumor formation or thrombosis compared with whole cell infusions, which is very attractive for aesthetic indications where long-term safety is paramount (Bian et al., 2022; Montero‐Vílchez et al., 2021; Jung et al., 2024; L et al., 2019).
Clinical and preclinical studies consistently describe favorable outcomes and good tolerability for MSC conditioned media in skin rejuvenation, scars, and hair growth (Zare et al., 2025; Zhu et al., 2025; Montero‐Vílchez et al., 2021).

Across regenerative and aesthetic indications, MSC-based and secretome-based treatments have shown reassuring safety in controlled settings, with adverse events typically mild (local swelling, bruising, transient discomfort) and serious complications rare when techniques follow best practice (Surowiecka & Strużyna, 2022; Bian et al., 2022; Gentile & Garcovich, 2021). Cell-free products are particularly promising for aesthetics because they retain potent regenerative signaling while naturally sidestepping many cell-related concerns such as engraftment, immune rejection, or uncontrolled proliferation (Bian et al., 2022; Montero‐Vílchez et al., 2021; Jung et al., 2024; L et al., 2019).
Current limitations, like variability in cell sources, preparation methods, and dosing, are best viewed as opportunities for optimization. Recent systematic reviews stress that standardizing manufacturing and characterization will likely make these therapies even more predictable, scalable, and safe for routine aesthetic practice (Wu et al., 2024; Zhu et al., 2025; Jung et al., 2024; Prado-Yupanqui et al., 2025; Aghayan et al., 2025). As protocols become more harmonized and larger clinical trials are completed, the strong regenerative and cosmetic potential already documented suggests MSC-based and especially MSC-derived, cell-free approaches can become a cornerstone of modern, biologically oriented aesthetic medicine.
